                  Report of Birth – which shall be filed first at the Philippine Embassy in Vienna, Austria; will be released after three (3) working days

  • PSA/NSO issued Marriage Certificate/Contract or Report of Marriage or PSA/NSO issued Report of Marriage of Parents: original and photocopy;
  • ePassport Fee: EUR 66.00;
  • Other documentary requirements, as deemed necessary.

 

RENEWAL OF RECENTLY EXPIRED/EXPIRING PASSPORT OF MINOR APPLICANTS (below 18 years old)

  • Personal appearance of the minor applicant
  • Personal appearance of the Filipino parent/s
  • Accomplished ePassport Application Form
  • Philippine Passport of the minor applicant: original and photocopy of the passport’s data page and last page
  • PSA/NSO issued Birth Certificate or Report of Birth of the minor applicant (original and photocopy)
  • PSA/NSO issued Marriage Certificate/Contract or Original Report of Marriage or PSA/NSO issued Report of Marriage, if the father is filing the minor’s application for the renewal of passport: original and photocopy;
  • Valid Philippine passport of the parent/s: original and photocopy
  • Valid visa – original and photocopy (front and back)
  • ePassport Fee: EUR 66.00
  • Other documentary requirements, as deemed necessary.

 

RENEWAL OF RECENTLY EXPIRED/EXPIRING PASSPORT

  • Personal appearance of the applicant
  • Accomplished ePassport Application Form
  • Philippine Passport of the applicant: original and photocopy of the data (information)  page and last page
    • If current passport has an AMENDMENT, submit a photocopy of the page showing amendment;
    • If passport in possession was issued before 01 May 1995, present the PSA/NSO issued birth certiticate
  • Valid visa – original and photocopy (front and back)
    • if with dual citizenship or with another foreign citizenship acquired through naturalization, original and photocopy of documents pertaining to re-acquisition/retention of Philippine citizenship (Oath of Allegiance, Identification Certificate, and Order of Re-Acquisition of Philippine Citizenship)
  • Report of Birth, if born with dual citizenship – original and photocopy
  • Other documentary requirements, as deemed necessary
  • ePassport Fee: EUR 66.00

 

RENEWAL OF RECENTLY EXPIRED/EXPIRING PASSPORT FOR WOMEN WHO WISH TO ADOPT THE MARRIED SURNAME (from single to married):

  • Personal appearance of the applicant;
  • Accomplished ePassport Application Form;
  • Philippine Passport of the applicant: original and photocopy of the passport’s data page and last page;
  • PSA issued Marriage Contract or Report of Marriage – original and photocopy; or
  • Valid visa – original and photocopy of the valid visa (front and back);
  • Other documentary requirements as deemed necessary
  • ePassport Fee: EUR 66.00

 

REPLACEMENT OF LOST OR DAMAGED e-PASSPORT

  • Personal appearance of the applicant;
  • Accomplished ePassport Application Form;
  • Copy of lost passport or remains of damaged passport;
  • Valid visa – original and photocopy of the valid visa (front and back);
  • DFA Authenticated PSA/NSO Birth Certificate or Report of Birth: original and photocopy;
  • DFA Authenticated PSA/NSO Marriage Certificate/Report of Marriage, if married: original and photocopy;
  • Affidavit of Loss or Damage stating in detail the circumstances of loss or damage, Affidavit Fee: EUR 22.50;
  • Police Report issued within the place where loss occurred with Official English translation. (For Lost passports only);
  • Other documentary requirements as deemed necessary
  • Lost ePassport Fee: EUR 165.00
  

IMPORTANT

  • Follow what is written in the birth certificate
  • Personal appearance is Mandatory;
  • Picture, Signature and Thumb printing shall be taken at the Philippine Embassy in Vienna;
  • During the picture taking:
    • Ears and eyebrows should be visible.
    • Applicant may smile but without showing teeth and gums. The "Mona Lisa" smile is recommended.
    • Eyeglasses, earrings, necklaces and hair accessories should always be removed.
    • Heavy makeup is not advised.
  • Other documentary requirements may be prescribed if the passport to be renewed is NOT an ePassport or has been expired for a considerable period, or has been mutilated, misplaced, etc. In this case, please inquire with the Philippine Embassy on the requirements.
  • The Embassy will not cancel the old passport until the applicant receives the new passport and will also return the cancelled old passport to the applicant.
  • The Embassy only accepts cash payment.
  • For applicants who wish to receive their new passport by mail, please bring an Aufgabeschein from the Austrian Post Office, self-addressed and self-stamped envelope:
    • Within Austria : EUR 6.50 worth of Austrian postage stamps;
    • Within Europe : EUR 12.15 cash only.
  • Kindly note that the new passports will not be mailed until the Embassy received the old passport for cancellation.
  • Applicants may visit viennape.dfa.gov.ph (Passports Ready for Release) to verify the availability of their new passport. Once availability has been confirmed, the applicant is advised to bring the old passport and receipt at the Embassy to claim the new passport. Or, the applicant may send the old passport and official receipt to the Embassy by mail. Upon the Embassy’s receipt of the old passport and official receipt, the old passport will be cancelled and will be mailed back to the applicant together with the new passport.
