REQUIREMENTS
- Report of Marriage Form – Five (5) originals, duly filled up and signed by both parties.
- Marriage Certificate - Original and five (5) photocopies
- ISSUED IN AUSTRIA :
- If the Heiratsurkunde is issued in Vienna, Apostilled by Landesgericht or from the Regional Court for Civil Law Matters (Justizpalast, Schmerlingplatz 10–11, 1010 Vienna);
- Must be translated by an official translator;
- English translation by an official translator shall only be done after the Apostillization;
- if the document is already in the English language, only the page with apostille shall be translated;
- Five (5) copies of all the pages
- ISSUED IN CROATIA / SLOVAK REPUBLIC / SLOVENIA :
- It must be APOSTILLED by the municipal court of jurisdiction over the place where the marriage certificate was issued, and/or Ministry of Justice whichever is applicable.
- After apostille, all pages must be translated by an official translator into English. If the document is already issued in English, only the apostille certificate/stamps must be translated.
- ISSUED IN AUSTRIA :
- Birth Certificates of husband and wife – Original and five (5) photocopies
- For Filipino spouse : Philippine Statistics Authority (PSA) Birth Certificate
- For Foreign spouse : Birth Certificate with English translation by an official translator
- Valid Passports of husband and wife - Original and five (5) photocopies
- For Filipino spouse : passport data page and last page
- For Foreign spouse : passport data page only
- PSA issued CENOMAR (Certificate of No Marriage) the CENOMAR issued by the PSA must not be more than six (6) months upon application – original and five (5) copies
- Fee: € 27.50
- Report of Marriage will be released after three (3) working days.
IMPORTANT REMINDERS
- In Report of Marriage form, wife’s column: please indicate her maiden/single name (refer to PSA birth certificate).
- Registration after one (1) year or more since the date of marriage: an Affidavit of Delayed Registration must be executed, and shall present a CENOMAR issued by PSA during the last six (6) months.
- A married Filipino woman who wishes to adopt her married name must present a PSA issued Marriage Contract or Report of Marriage.
Additional documents shall be required for the following cases:
- Annulment: Court approved annulment documents + Certificate of Finality and PSA issued marriage contract/certificate with annotation on annulment - original + 5 copies.
- Divorce: Court decision recognizing the divorce + PSA issued marriage contract/certificate with annotation on court recognition of divorce - original + 5 copies.
- Widow/er: PSA issued death certificate - original + 5 copies.
