REQUIREMENTS

1. Report of Birth form - To be accomplished in five (5) originals, signed by the Filipino Parent
2. Birth Certificate of the applicant – Original and Five (5) copies
   • IF BORN IN AUSTRIA :
     • GEBURTSURKUNDE with middle name (mittel name)
     • Must be translated by an official translator;
     • If the Geburtsurkunde is issued in Vienna, Apostilled by Landesgericht or from the Regional Court for Civil Law Matters (Justizpalast, Schmerlingplatz 10–11, 1010 Vienna);
     • English translation by an official translator shall only be done after the Apostillization;
     • if the document is already in the English language, only the page with apostille shall be translated;
     • Five (5) copies of all the pages
   • IF BORN IN CROATIA :
     • it must be apostilled by the Croatian Ministry of Justice
     • it must be translated into the English language by an official translator
     • if the document is already in the English language, only the page with apostille shall be translated
     • The maiden/single surname of the mother must be indicated
     • Five (5) copies of all the pages
   • IF BORN IN THE SLOVAK REPUBLIC :
     • it must be apostilled by the Slovak Ministry of Justice
     • it must be translated into the English language by an official translator
     • if the document is already in the English language, only the page with apostille shall be translated
     • The maiden/single surname of the mother must be indicated
     • Five (5) copies of all the pages
   • IF BORN IN SLOVENIA :
     • it must be apostilled by the Slovenian Ministry of Justice
     • it must be translated into the English language by an official translator
     • if the document is already in the English language, only the page with apostille shall be translated
     • The maiden/single surname of the mother must be indicated
     • Five (5) copies of all the pages
3. PSA Marriage Contract or PSA Report of Marriage (if parents are married) - Original and five (5) copies
4. Birth Certificate of Parents – Original and five (5) copies
   • For Filipino parent/s – Philippine Statistics Authority (PSA) Birth Certificate
   • For Foreign parent – Birth Certificate with English translation by an official translator
5. Parent’s latest valid passports - Original
   • Five (5) copies of mother’s valid passport (data page and last page)
   • Five (5) copies of father’s valid passport (data page and last page)
6. Fee: € 27.50
7. Application for Report of Birth will be released after three (3) working days.

IMPORTANT REMINDERS

1. Personal appearance of the Filipino parent.
2. Entries in the Report of Birth must be at time of birth of child.
3. For Late Registration:
   
   • Please submit a copy of parent's passports at the time of child's birth. If not applicable, any valid documents as proof of parent's Philippine citizenship at the time of child's birth.
   • If the report is being made one (1) year after the birth of the child, a Certificate of No Record of Birth issued by the PSA, issued not more than six (6) months upon filing of Report of Birth, is required, as well as the execution of an Affidavit of Delayed Registration.
4. For Illegitimate Children:  
   
   • For those whose parents are not married but there is no impediment for them to get married;
   • An Affidavit of Admission of Paternity shall be filed by the father and an Affidavit to Use the Surname of the Father shall be filed by the mother if the child will use the father's surname and the name of the father is indicated in the child's local birth certificate. Must be accomplished in five (5) original copies.
5. An affidavit costs € 27.50. As with all affidavits, a copy of the affiant’s valid passport must be attached.
6. In filling up the Report of Birth Form, Item No. 10 shall reflect the complete name of the foreign parent.
